Medical History Form

  1. Log into the GMercyU Portal. 
  2. Click the Student Wellness Portal icon to launch our Student Wellness Portal.

Required Immunizations

Insurance Coverage

February 2024 update

All full-time campus undergraduate students, ABSN, DEMSN, MSOT, 学生必须购买医疗保险. 住校学生须在健康表上提供健康保险证明，方可在校住宿.

Meningitis Awareness Disclosure
